Card Investigation Appointment

If you are a victim of card cloning, you must immediately send us your compromised card so we can begin our investigation.

You may cut the card in half and place both pieces in an envelope. Include a note with your case number and current PIN. However, do not damage the chip, as it is required for data analysis and scanning.

Please complete the form as soon as possible. Once submitted, we will dispatch an authorized driver to collect the compromised card. A replacement card will be issued within 3–5 business days.
